I'm using Real Guitar 3.
Is it possible to edit the down strum velocity/accent and if so how?

Not clear... Do you want to edit down strum velocity in a pattern?

To control pattern strokes velocity on the fly you can use Pitch Bender controller (activate P.B. LED on Pattern window)
To edit MIDI pattern: drag it from Pattern view to your host's MIDI track, change velocity of notes/strokes in Piano Roll editor, and then save edited pattern as Standard MIDI file type 0 (single track) to User subfolder in Loops folder found in your RealGuitar installation directory. After that close-run RealGuitar and you'll find your pattern in Pattern window/User folder
