you can do this using Chainer, it also works for any other VSTi instrument ou there.

elbaby765 is right. Chainer is exellent for this application. And there is a free version. . . only the saving and loading of presets are disabled.
Other hosts and sequencer/DAW's can do this as well, like Sonar, Cubase, EnergyXT, Abelton Live, VStack, etc., but those will set you back some cash.
The bottom line is, you'll need a VST host that allows chaining of plugins. Stand alone mode of Real Guitar has no way of routing it's audio through effects unless you send the audio out of your sound card to an external effects processor.

I've used it through Cakewalk and have run it through amp sims, flanging, etc.
I've been able to get an Early British type of sound out of it using verb and a light crunch effect on the amp sim, but I've only begun to experiment. I think the possibilitites are pretty endless.

Chainer is cool but xLutop doesn't support it anymore..
Check out Brainspawn's Forte. It's really cool!!!
