It appears that the glitch occurs if you happen to play the same note repeatedly too quickly in succession. This glitch is easy to show.... just pay the same note again and again quickly until the note hangs. Unfortunately, this can be triggered in normal play without realizing that you have repeated the same note too soon after the last note on message. Perhaps there are other reasons why the notes hang but if you accurately alternate between two notes, they don't hang.
